Wednesdays from 5 p.m. to 6 p.m.

Registration
Register Monday by 2 p.m. If you submit or reserve after this time, please call the church office to verify there will be availability of food for your reservation. If you need to change your order, you can resubmit this form or call the church office at 757-539-6629.

Instructions
Please choose meals according to your schedule either for the month, or only one week by placing a 0 value in the meal option. Please enter in the comments box any additional notes to the dates you choose.

Meal/Salad
Meal: an entree, two vegetables, roll, and dessert. 
Salad: a full-size chef salad includes lettuce, ham/turkey, egg, and vegetables.

Cost
Adult Meal: $5/pp
Child (4-11): $3/pp
Child (3 and under): Free
Take-out box: $1/ea
No Family Max
